I ) Instructions :

1. Please complete the form appearing on the page after you click the " I Agree " button fully. All fields marked with * are compulsory.
2. After verifying that you have filled the form correctly and fully, click on the submit button at the bottom to submit the form. Please submit a form only once.
3. Printout the summary page presented to you on the next screen after clicking submit button.
4. Bring the printout to the college for document scrutiny & submission with necessary documents as per list below.
5. Prospectus will be issued at the time of submission of the form and document at the college.
6. Processing fees of Rs. 100/- to be paid at the time of submission of this form.
7. This form will be considered for admission only if you have submitted the printout along with necessary document copies AND paid the processing fees.

1. Marksheet of last exam passed
2. Passing Certificate of last exam passed
3. Address proof
4. Any other relevant documents
5. 2 passport size Color Photographs
