Online Admission Form for First Year Diploma.

Kindly note the following points before filling the online admission form.

I ) Instructions :

1. Please complete the form appearing on the page after you click the " I Agree " button fully. All fields marked with * are compulsory.
2. After verifying that you have filled the form correctly and fully, click on the submit button at the bottom to submit the form. Please submit a form only once.
3. Printout the summary page presented to you on the next screen after clicking submit button.
4. Bring the printout to the college for document scrutiny & submission with attested copies of all necessary documents as per list below.
5. Prospectus will be issued at the time of submission of the form and documents at the Polytechnic.
6. Processing fees of Rs. 200/- ( Rs. 100/- for reserve category ) to be paid at the time of submission of this form.
7. This form will be considered for admission only if you have submitted the printout along with necessary document copies AND paid the processing fees before the last date.

II ) Eligibility :

• Reserved Category : - S.S.C. or Equivalent passed with minimum 45% Marks. e.g. minimum 292.5 marks out of 650..
• Open Category : - S.S.C. or Equivalent passed with minimum 50% Marks. e.g. minimum 325 marks out of 650.

III ) Documents to be submitted :

    • Open Category
      Two attested photocopies of the following..

1. 10th Marksheet
2. School / College Leaving Certificate
3. Affidavit for gap period if any.
4. 12th Marksheet (H.S.C. passed student)

   • Reserved Category should submit following additional documents
      Three attested photocopies of the following..

5. Caste Certificate of students
6. Non-creamy layer certificate from collector for the year 2008-09 for OBC, SBC, NT-B, NT-C, NT-D, VJDT.
7. Photocopy of Ration Card.
8. Caste Validity certificate for ST category.
9. Gross Annual Income certificate 2008-09 for lower Income group i.e. Below Rupees One lakh per annum (for 2008-09) / Annual Income certificate / (Form-16) for 2008-09 / Income Tax return / Certificate for Salary / Self employed Parents.

   • Physically Handicapped candidates Medical Certificate from civil surgeon

   • Defense Service Personel

• Certificate from Zilha Sainik Welfare Officer for (Ex-Defence) or from Commandant or equivalent authority for active service personel..
